Forms, certificates and medicals

Completion of forms, certificates and medicals are not covered under the NHS. Payment for reports must accompany the request or appointment booking (i.e. in advance).

Charges are aligned with the BMA’s guidelines. Please read the form carefully and fill in and sign any parts that you need to complete before you bring the form to the surgery.

Please allow up to 5 to 10 working days (this maybe longer during periods of GP annual leave or illness) for the process to be completed.

Heavy Goods Vehicle Medicals

Heavy Good Vehicle (HGV) Medicals can be completed by either Doctor Murdoch or Doctor Mackie.  These medicals are required for anyone who drives Heavy Good Vehicles, and are only available to patients of the practice.  These medicals take around 1 hour, and are priced at £130, payable to the practice by either cash or cheque.

ENG1 Medicals

The ENG1 is a basic medical carried out by Doctor Mackie who is the practice specialist in this area.  The Medical is for both patients of the practice and private patients who work within the seafarers profession. It involves an appointment with our Practice Nurse and Doctor Mackie, which will take around 1 hour.  The ENG1 is priced at £115, payable to the practice by either cash or cheque.

Offshore Medicals

The Offshore is a similar medical to the ENG1, again carried out by Doctor Mackie.  This medical is also for patients of the practice and private patients who work Offshore.  This appointment also involves an appointment with our Practice Nurse and Doctor Mackie, this again will take around 1 hour.  The Offshore Medical is priced at £150, payable to the practice by either cash or cheque.
