Their advanced training and skills enable them to care for people in their own homes. Each referred patient is assessed individually and a comprehensive range of nursing care is available to suit his or her needs. These range from administering medication to looking after people in the terminal stages of illness, as well as offering counseling and support to families and carers. Health Visitors are available to all age groups and are there to provide help and support to individuals and families both within the surgery and home setting. They are committed to promoting and improving emotional, physical and mental health for all patients in the practice.
